Notes:

1. Like the worksheets, these come in many different formats and will have to be designed individually. Some are regular tables, while others are fill-in charts for the reader. The latter should perhaps retain the boxes, as shown in the manuscript, as I think these are meant to make filling in the blanks easier. Otherwise perhaps underscores of appropriate width could be used where the answers are to be filled in.

2. I started to call out the table bodies by number of columns, but then I noticed that tables with the same number of columns often had very different spatial divisions, so I have just called them out as TB, as they will have to be spec'ed individually.

3. Some of the tables have TH, some don't. Note that in each table called a "Bit-by-Bit Budget," that phrase should be a TH, not a TCH as typed. These appear on pp. 26, 52, 112, 152, 206, 268, 358, 415, and 550.
